Liza Hoffman

Liza Hoffman, LICSW, serves as a Senior Program Manager at Community Care Cooperative (C3), a 501c3 non-profit Accountable Care Organization that is founded, owned, and operated by Federally Qualified Health Centers (FQHCs). As a member of the Primary Care Innovation team, she leads strategic initiatives to advance the effective, equitable, and responsible adoption of AI technology across C3’s national network of community health centers. 

Throughout her career, Liza has designed and implemented innovative technology solutions and care models aimed at increasing access to care, improving operational efficiency, and reducing clinician burnout. She has held leadership roles spanning telehealth ventures, research institutions, and safety-net health systems, with expertise in digital health innovation, integrated behavioral health, and user experience research. 

 At Cambridge Health Alliance, Liza helped lead the design and implementation of a new Primary Care behavioral health integration program; she then launched a digital literacy program for underserved populations while serving as the Clinical Director of the Beth Israel Deaconess Medical Center Division of Digital Psychiatry. At the onset of the Covid 19 pandemic, Liza led efforts across clinical, operational, and product functions to safely scale-up a fully-remote Medication for Opioid Use Disorder treatment program. 

As a digital health consultant, Liza provides subject matter expertise for both federal/state initiatives as well as early-stage healthtech ventures. She is also a guest lecturer for graduate-level programs and has presented at national conferences including the American Psychiatric Association, SAMHSA, and Harvard Medical School.
