Kati Moran, MPH is the Director of Primary Care Innovation at Community Care Cooperative (C3). In this role, she works with C3’s member FQHCs to strengthen primary care delivery and build the capabilities needed to succeed in value-based care. In this role, she partners with FQHCs to design, test, and scale innovative care models, advance care team redesign, and support practice transformation efforts that improve both patient outcomes and care team experience. Her work also includes exploring digital health solutions and technology-enabled approaches to improve access to care and support care team well-being. Prior to this role, she held other positions at C3 in the areas of Performance Improvement, Telehealth, and Quality. Kati holds a Bachelor of Science in Health Science and a Master of Public Health from Boston University
